I have included in this blog post a detailed list of necessary information and documents that you will need in order to apply for social security disability. You will need:
- Your Social Security number.
- Names, addresses and phone numbers of the doctors, caseworkers, hospitals and clinics that took care of you and dates of your visits.
- Medical records from your doctors, therapists, counselors, hospitals, clinics and caseworkers that you already have in your possession.
- Laboratory and test results that you have in your possession.
- Workers’ compensation contact names, addresses and dates.
- Dates of marriages and divorces, spouses’ birthdates and social security numbers.
- Date you became disabled- not necessarily the last day that you worked.
- Names and dosage of all the medicine you take- here is a form you can use to list your medications:
- A copy of your most recent W-2 Form (Wage and Tax Statement) or, if you are self-employed, your federal tax return for the past year.
- Your certified birth or baptismal certificate- a copy will not be accepted.
